Prize Details of Walmart Survey for Customers

There will be two types of prizes rewarded to the Walmart customers:

Grand prizes: Five (5) grand prizes will be awarded, each for $1,000 Walmart Gift Cards (the “Grand Prizes”). A retail value of $5,000 is approximate for all Grand Prizes.

First Prizes: A $100 Walmart gift card will be the first prize for each of the 750 participants. Each of these is called a “First Prize.” All First Prizes have a total ARV of $75,000.

Total Prize: All of the prizes have a total ARV of $80,000.

Grand Prize

On or around November 7, 2022 (the “Notification”), the Independent Administrator will contact potential Grand Prize winners by email or phone. In order to claim the Grand Prize, winners must respond within five business days of receiving the first notification from the Independent Administrator.

After five business days from the Independent Administrator’s first notification to the Potential Prize Winner, if he or she does not respond to the Notification, or if the Sponsor and Independent Administrator, in their sole discretion, determine a potential Prize Winner is ineligible, an alternate Prize Winner will be drawn from the remaining non-winning eligible entries.

An Affidavit of Eligibility and Liability/Publicity Release, as well as a W-9 tax form (the “Paperwork”), are required from the date of notification. Parents or legal guardians must sign the Paperwork on behalf of Grand Prize winners who are minors where they live (but who are at least 18 years old).

Parents or legal guardians will receive the Grand Prize on their behalf. The Grand Prize will be sent by Priority Mail through the United States Postal Service after reviewing the Affidavit.

Those who may win the Grand Prize must return the Paperwork by the date and time specified in the notification. An entry will be disqualified if the potential Grand Prize winner does not return the Paperwork by the date and time specified in the Notification, is found ineligible, or does not follow the Official Rules.

Another potential Grand Prize winner will be selected from the remaining eligible entries. The entry will be thrown out if the Notification is returned as undeliverable, and a new Grand Prize winner will be chosen from all other eligible entries.

Grand Prize winners are solely responsible for any taxes due to the value of the Grand Prize, including but not limited to federal, state, and local taxes.

In the year the Grand Prize is given to the Grand Prize winner, the Sponsor must notify the Internal Revenue Service of the prize’s value. Form 1099 will be issued to the Grand Prize winner and the Internal Revenue Service.

After signing the paperwork, Grand Prize winners will receive their prize within four weeks.

Upon winning a First Prize, potential winners will receive it at their address on file with the Sponsor or the address they provided at the time of entry within 4 to 6 weeks of the drawing. First Prize winners may be disqualified if the prize is returned, if the address is too short or too long for shipping purposes, or if it cannot be delivered for any other reason.

A new First Prize winner will be selected from the remaining eligible entries. An alternate First Prize winner will be disqualified, and the First Prize will not be awarded if their First Prize is returned, if their address is insufficient or incomplete for shipping purposes, or if the prize cannot be delivered.

No compensation for prizes that can’t be given to a winner will be given.
